You may have sun-derived or post-inflammatory hyperpigmentation (PIH) if the areas of pigmentation are like freckles, are more apparent with sun exposure and not usually symmetrical on the face. They can appear as a larger cluster of pigmentation in some areas.

Inflammation from the sun, or chemical peels and lasers, can result in a wound that does not heal. Especially if the injury is severe or results from repetitive wounding as is the case in most age spots. The wound does not heal for a variety of reasons and often appears as a small red spot on the skin. Age spots and PIH are simply the body making an "umbrella" of protection using melanin as its tool.
